Need to Know

Before you arrive, speed up your check-in process by completing the two forms below. It is important that your medical history form is completed accurately and to the best of your knowledge. (We do have them at the clinic in case you don't have them with you).

If you wear any glasses or contacts, please be sure to bring those. Otherwise, we will need you to retrieve them and return to the clinic to complete your physical.

Appointments or Walk-In?

We are currently accepting walk-ins, however, emergent situations will take precedence and you may experience a longer wait time if we have patients already in the clinic.

You may schedule an appointment, however, this is just a courtesy to let us know when you'll be arriving at the clinic as we will continue to accept walk-ins. You may experience a slight wait if we are assisting other patients at the time of your visit.

Sickle Cell Testing

We do offer Sickle Cell Testing, however, this will require you to receive it at an outside Diagnostic Laboratory Services (DLS) location, we will provide you with locations at the time of your visit and results vary between 1-3 days.
